You will be workingalongside a multidisciplinary independence team which includes SocialWorkers / Social Care Assessors and Occupational Therapists with a clear focus onReablement.
As part of theIndependence, Reablement team you will be responsible for preventing, reducingand delaying the needs of the adults with care and support needs and theircarers.
Working with people in the community and hospitals you will providesimple assessment advice, information and guidance around non-complexequipment, adaptations, technology and telecare solutions, making referrals tothe independence team where appropriate.
You will completesimple assessments both in the community and the hospitals looking at the careand support needs of the person and where appropriate offering the Reablementservice to enable the person to meet their agreed outcomes and goals, promotingand maximising independence and again looking at equipment and assistivetechnology to support this.
Information gathered will be recorded within LLA,ensuring this is accurate and timely. You will have a minimum of a Level 2 inHealth and Social Care or QCF Level 2 diploma in Health and Social Care, withthe service committed to give you the ongoing support and training to achieveLevel 3 to aid your professional development.
