We have an exciting opportunity to offer a Band 3 HCSW to join our Older Adult Community Mental Health Team within the East Lancashire / Pennine locality, based at Balladen House, Rawtenstall, overlooking the beautiful Rossendale Valley.
The service provides assessment and treatment to service users referred with a variety of mental health conditions within the community setting, including those identified with Dementia.
The team is multi-disciplinary and works closely with other mental health services and teams.
Potential candidates should ideally have relevant Older Adult / Adult Mental Health community experience and have proven skills within Community mental health services.
We are looking for a candidate who has excellent skills, communication and knowledge, and a positive attitude to the care of Older Adults within the community setting.
The ability to identify and report risk in line with dedicated care plans is essential, be able to liaise and communicate effectively with the wider care team / professional lead, and have a working knowledge of the dedicated care planning process.
Candidates will be able to demonstrate resilience, and have the ability to manage the demands of a busy community team. Be able to demonstrate Trust values in line with role and expectations.
Be able to offer support in line with competencies to others, and be active in supporting student activity.
There is a robust supervision and personal development structure in place via the team manager and the successful candidate will have development opportunities in house and externally.
