The Children & FamilyWellbeing Service enables children and families to access appropriate supportas early as possible, to help them maintain their quality of life, prevent anyproblems getting worse and reduce the demand for specialist support services.
Weare seeking to appoint a Key Worker who will both, carry a caseload of clientsand design and participate in group based delivery programmes at one or moreneighbourhood centres in their area.
Thepostholder will have involvement in programme monitoring particularly aroundthe quality of programmes and tasks associated with programme planning, reviewand accountability.
Ascase holders, Key Workers will undertake appropriate assessment of the needs oftheir allocated client group in line with agreed Common Assessment Framework(CAF) processes and progress appropriate responses in line with 'team aroundthe family' methodology.
KeyWorkers will undertake a lead professional role where appropriate in caseswhich require multi-agency support for families / individuals and will developand deliver smart action plans designed to address identified needs and useagreed tools to measure the progress made by families / individuals towardssubstantial and sustained change e.g. outcome stars.
Someinvolvement in evening and weekend delivery sessions may be required.
QualificationDetails : Level3 or above professional and / or relevant academic qualification ORsubstantial vocational experience in a relevant technical or operational field.
