Supported Living Coordinator CQCServices Exeter Devon Permanent, Full Time (37.5 hpw)

Salary £19,000 to £20,250 pa rising to£24,000 through length of service

We would like you to join our fab andsupportive team. Our Devon Enhanced Community Recovery Service is basedacross different locations within Exeter & Torquay.

We support the recoveryof our customers, who are generally over 18 years old, and help them to gainindependence within the community and to build on the support networks andskills to live fulfilling lives.

Our service provides a person centeredsupport package that supports customers in their own living environments, tolearn and develop life skills, manage their own risks and outcomes with the aimbeing for them to move on to more independent living.

Our customers areunique, and very different in their own ways and in how we support them, butthey all have varying mental health support needs! As a Supported Living Coordinator,you'll be supporting our clients in their home offering emotional and mentalsupport.

Our customers live in a 5 bedroomed detached house inExeter.

You’ll help our customers identifytheir aspirations, and work with them to create plans that help them achievethese. You also help your colleagues to deliver on the plans you’ve created toachieve excellence.

Our aim is to support our customers to live their day today lives as independently as possible. The team will tell you that no twodays (or nights) are ever the same because everything we do is personcentred , around the needs of our customers.

About our team

Our team describe themselves as reallyfriendly and approachable. In fact, when new members have joined us in thepast, it’s been said that they felt welcomed by colleagues and our customersfrom day one! We’re also super passionate about supporting customers ourteam are tight and really have each other's backs, even though we are splitacross different locations.

Many of our team have progressed their careerswithin Home Group. Becky our Registered Manager has worked here for 6 years,and her, Adam and Geoff have all become managers, having started in otherroles.

Our Clinical Lead Ian has only been with us for a few years but lovesthe transition he has made to join us from the NHS.

We are also investing inour team’s clinical knowledge and experience, so when you join us there is lotsof scope for personal growth and development.

But if you are happy doing yourown thing then that is great too!

Working hours

We provide 24-hour support to ourcustomers each day and every day. There are two roles : in one role you’ll workdaytime hours and in the other role you’ll work during the twilight hours doingwaking nights.

Looking for a more flexible workingpattern? You can also join us on a flexible basis by being part of our bank.You can work hours that fit around your current commitments or even pick upsome additional shifts to your main role to earn some extra cash and get paidon a weekly basis.

About you

You’re passionate and motivated. To youproviding care and support is more than just a job, you make every customerfeel valued and respected.

You’re compassionate and use your unique empathy andcreativity skills to help our customers reach their full potential. Treatingyour customers with integrity, you ensure that they maintain their dignity atall times.

You’ll help change our customer’s lives(and our services) for the better by undertaking risk assessments, supportplanning, goal setting and regular reviews.

You’ll create bespoke andpersonalized support plans with and for your customers. Each planwill look to meet our customer’s individual potential and long-term goals,making sure they meet safeguarding and confidentiality requirements.

You’llalso have responsibility for coordinating our support workers, apprentices andvolunteers, and working collaboratively with external agencies, to assess andengage support for our customers.

It would be amazing if you have workedin this type of environment before, however it’s your can do positiveattitude and enthusiasm that are critical to make this role a success.

As this is a community-based role,you’ll need to travel around to spend time with your customers. You could dothis role using public transport, or if you drive and have access to a car thatwould also be amazing! (We pay your expenses to cover this use!)

When you join, you’ll need to have avalid Enhanced Check under the Disclosure and Barring Service. The great newsis that we will pay for that to be done if you don’t have a transferrable one.
