First City Nursing Services have a great temporary opportunity for someone to join our Support Planning Team as an integral and vital role in the business.

This is the perfect role for an existing Care Supervisor or a Care Assistant looking to progress their career.

The Temporary Support Planner is responsible for undertaking the initial client assessments, setting up the care delivery for each client, and ensuring the service that's delivered is consistently strong.

The role involves visiting clients to create a tailored care and support plan which is unique to them.

The Temporary Support Planner needs to be compassionate, self-assured and have the knowledge and ability to model good practice whilst being prepared to challenge poor performance.

You will regularly monitor the quality of the care that your service provides and take action to make improvements through regular reviews and risk assessments.

For this role, you must be experienced within the care sector and hold a minimum of NVQ 2 or equivalent.

Key Skills you should possess :

  • Experience with writing person-centred, outcome-focused support plans
  • Signposting and making referrals to those receiving the service
  • Experience with writing up MAR Charts
  • Experience in auditing Daily Log sheets and MAR Charts
  • Ability to time manage, be proactive and work on your own initiative
  • Professional manner and excellent customer service when liaising with other health care professionals and clients
  • Openness to attend social services reviews and supervisions
  • Friendly and supportive toward vulnerable adults and staff
  • An excellent team player
  • Possess good IT skills (including the use of Microsoft Office packages) and have a good telephone manner
  • Be able to assess clients for individual needs and carry out risk assessments
  • Reliable and flexible
  • Excellent communications skills and the ability to build relationships with the clients we support and those around them
  • Strong leadership and communication skills
  • Ability to be patient and keep calm under pressure
  • Confidence to conduct client reviews and respond to Individual Review Forms completed by Care & Support Workers as soon as possible.
  • Ability to urgently report any changes in a Care & Support Worker's or client's schedule to the Care Coordinator.
  • Hours : Monday to Friday 08.30 - 17.00pm

    20 days A / L + BH

    Driving licence and use of personal vehicle for work purposes is essential for this role.

    All employment with First City Nursing and Care subject to satisfactory references, enhanced DBS and full completion of mandatory training.
