We are delighted to welcome applications to join the Frail Elderly Service (FES) as a Clinical Support Worker based at the Manor Hospital, Walsall.

The care of older people living with frailty has become a health service priority in recent years. A person living with frailty may suffer dramatic effects on their health and physical function as a consequence of small changes brought on by illness or injury.

The care that they receive in hospital can make a major difference to their recovery and return to independence.

Main duties of the job

The Frailty Service uses multidisciplinary working to provide high quality holistic assessment and care planning for this vulnerable group of patients.

We strive to provide a person centred approach and work closely with patients, carers and community services to improve overall wellbeing and either avoid hospital admission or reduce the length of stay required.

Our work requires lots of compassion, problem solving and teamwork. It is often highly challenging but is also highly rewarding.

Successful applicants will be actively encouraged to develop their skills and knowledge to meet both personal and service development needs.

Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading.

Detailed job description and main responsibilities

You will form a vital part of this busy and dedicated multidisciplinary team (MDT). Some of your responsibilities will include carrying out initial assessments of arriving patients, assisting patients with pastoral care and communicating closely with other members of the MDT to help in decision making.

Essential criteria

  • Qualification at NVQ 3 relevant to health or social care
  • Essential criteria

  • Previous experience as a support worker in an acute or community setting
  • Specific experience of working with older adults in a health or social care environment
  • Experience of independently delivering patient care
  • Experience of working within a multidisciplanry team
  • Experience of complex discharge planning incorporating health and social care pathways
  • Essential criteria

  • Good verbal and written communication skills
  • Basic computer skills
  • Ability to independently obrain clinical observations
  • Ability to perform basic clinical interventions such as venepuncture and ECG acquisition
  • Ability to recognise and respond appropriately to changes in patient condition and inform an appropriate person

  • Understanding of the specifice needs of older people living with frailty
  • Good understanding of the principles and practice of clinical governance
  • Knowledge and understanding of clinical nursing practice and an ability to support in its provision
