Oxfordshire’s Children’s Continuing Health Care team is delighted to advertise an opportunity for an enthusiastic nurse to join the team in a leadership role.

As a member of this creative, friendly team you will provide clinical leadership in order to support the assessment and ongoing monitoring of young people with complex physical health needs who meet the eligibility criteria for the NHS National Framework for Children and Young People’s Continuing Care.

In your role, alongside excellent assessment and co-ordination skills we are looking for individuals with a strong commitment to partnership working in order to ensure our families receive full care provision according to individual needs.

Each package varies according to need and delivery of health care and we are looking for individuals able to think with and support families to achieve their own individual positive health outcomes.

We are a busy team often working away from Base and so the post holder will need to feel comfortable with a high level of autonomy, be skilled at managing conflicting priorities as well as confident in their practice.

Flexibility in working pattern to suit service needs will be required.
