The Team consists of Health and Wellbeing Assessors, Social Workers, Experienced Social Workers and Advanced Social Workers
In the context of developing our services to continue to meet the challenges of the Care Act 2014, the Croydon Council People Department is restructuring its services and we are currently looking for social workers at various levels to join our Locality Adult Social Care Teams.
We offer a supportive working environment for individuals to further develop their knowledge and practice within crisis intervention short term duty team.
Key Responsibilities :
To undertake Strength / Asset Based / Assessments and or reviews, sign post, respond to enquiries and to implement support packages where required to minimise the risks identified In line with the Car Act 2019 and Community Led Support.
Participate in the team duty rota to undertake over the phone risk assessments and at times to respond to a crisis situation and implement any support necessary to minimise the risks identified and ensure practice is underpinned by the principles of the Care Act 2014 and Community Led Support.
To triage safeguarding initial concerns raised, information gathering and participating in SAM discussions to make decisions as to whether concerns raised need a formal s.
42 or pass to care management.
Work in partnership with service users, carers and family members
Ensure a high standard of case recording
To carry out welfare checks in response to safeguarding concern being raised in an urgent situation.
Complete risk assessments and implement protection plans as required as part of crisis intervention role and responsibilities.
Undertake Mental Capacity Assessments when required
Complete referrals to Advocacy services
Complete Court reports as required and or attend Court.
Manage and prioritise own caseload

Applicants will demonstrate exceptionally strong social work practice skills and sound knowledge of key legislation and aspirations to extend their practice experience and skills.
Candidates at this level will be expected to show evidence of completion of relevant post-qualification award(s), as well as capacity to take on some leadership and supervisory responsibilities around safeguarding triaging.
Appointed applicants will share in supporting their Manager's leadership of the team to deliver progressive services and promote and maintain high standards of practice.
