Advanced Clinical Practitioner, Exeter Exe CMHT
We are seeking to recruit an enthusiastic, highly motivated and experienced Advanced Clinical Practitioner to work alongside an Associate Specialist doctor and Multi-Disciplinary Team in the Exe CMHT, one of 3 general adult psychiatry CMHTs covering the city of Exeter.
The definition of Advanced Clinical Practice in England is as follows :
Advanced Clinical Practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making.
This is underpinned by a masters level award or equivalent that encompasses the four pillars of advanced practice :
The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced person who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
They will demonstrate safe, competent clinical decision-making and expert care. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process.
They will communicate and work collaboratively with the team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership as required.
The post holder will work as a member of the multi professional team by providing an advanced clinical role to prevent un-necessary acute hospital admissions and where safe, appropriate and in the persons best interest to do so keep patients in their own homes for longer, work to identify and proactively prevent premature relapse and / or deterioration in a person’s condition hence where possible avoid admissions into long term care facilities.
The post holder will work as part of a multi professional team providing a clinical service that is sensitive to the needs of the population.
The team will provide comprehensive assessment in order to establish diagnosis and creation of an appropriate, person centred, evidenced based treatment plan.
The post holder will not carry a caseload. They will be expected to be available to review and provide advice complex patients on behalf of the MDT as well as providing advice to primary care and other stakeholders.
The Advanced Practitioners will contribute to the development and implementation of effective models of care to facilitate timely, clinician to clinician advice.
Salary : £31,696 £41,787
