Job Title : Service Manager

Salary Range : Hay C £54,461 to £57,637

Hours per week : 37 hours

Contract type : Permanent

The Combined Placement and Sourcing Team (CPST) currently has two main roles :

  • It is responsible for sourcing care and support at home, and care home placements, for people living in the community who are the responsibility of adult social care.
  • Managing referrals for most discharge pathways from both acute and community hospital settings, on behalf of both social care and health.
  • The team’s function is to source appropriate placements or care packages to support all patients to return home or enter an appropriate bedded environment, always with a focus on people maximising their independence.

    You will be joining the team at an exciting time of transformation within our approach to Health and Social Care integration.

    Created via the COVID Discharge guidance, the CPST needs to plan, develop and lead the provision of support across all stakeholders who provide support and bed spaces.

    This will move further into admission avoidance services and end of life care. The post-holder will have significant interaction with partners working on high profile, sometimes politically sensitive projects (for example, Adult Services transformation and the Community Response and Reablement Programme)

    The role will manage a set of teams and resources, being both operational and strategic support in nature, often requiring co-ordination with other teams and closely related functions.

    This will include the performance management of the service responsibility for the development of the future shape of service provision.

    The post-holder will build relationships and influence activities, including working across and outside of the organisation to improve services.

    To be successful in the role, you will have great operational and strategic skills, often acting as the link between senior managers in Health and Social Care and the delivery teams that report into you.

    You will be confident in interpreting and existing and new legislation and guidance, both nationally and locally. In addition to this, you will have a working knowledge of Care Act legislation, assessments, preparation of support plans, monitoring and reviewing outcomes as well as clear understanding of safeguarding.
