This new role within East Sussex Healthcare Community Services (CHS) pharmacy team, one of a two handed team, has been created following a service review.
We are a small team within an integrated acute and community trust.
We are seeking an experienced, enthusiastic, self-motivated and dynamic Pharmacist with excellent communication and interpersonal skills to provide a pharmacy service to our community-based services.
The postholder will be working with a dedicated pharmacy technician, and other members of the CHS pharmacy team.
This post will have a focus on the medicines related needs of our community based services (excluding bedded units) to provide advice and support on safe handling of medicines in the broadest sense, extending into the development of services supporting medicines optimisation in the patient’s home, working with your community healthcare colleagues.
You will have a good understanding of the services delivered, and challenges met, in the community setting, and how different agencies contribute to the picture.
This role will range through a spectrum of medicines use, from medicines administration and associated governance, through policy development and implementation, to specialist pharmacist advice for service development.
Safe patient care will underpin all that you do.
You will have a vision of how you can incorporate medicines optimisation to become an integral part of the community based services, supporting safe use of medicines for the patient cohort served by our community based services.
You will have a desire to create a service that meets the needs of both the clinical staff and the patients they care for.
