NHS Forth Valley seeks to appoint enthusiastic, forward looking GPs to join our expanding Closer to Home Team of dedicated doctors, nurses and HCAs.

We have close links to community nursing and rehab teams, social services, Ageing and Health Consultants and General Practice.

The team is focussed on providing high quality care to allow patients, where appropriate, to receive acute medical and nursing care at home, where previously they may have required hospital admission.

It sits in the intermediate area between traditional primary care and secondary care and is constantly evolving and developing.

Recent developments of the service include support for uninjured fallers, a COPD exacerbation pilot and work to support early discharge to intermediate care beds.

One of the team has been working with Ageing and Health to pilot the RESPECT (Recommended Summary Plan for Emergency Care and Treatment) documentation in the community.

The new Stirling Care Village will help provide further areas for development with the potential to have better access to step up beds.

GP practice placements, if desired, can also be made available in traditional 17j practices or our 2c Board Run practices, which are at the leading edge of Primary Care Service Development, exploring new models of working, using an expanded multi-

disciplinary team to support patient care. These have on site pharmacists, ANPs, practice nurses, Mental Health Nurses and ESPs (physiotherapists) provide an enhanced experience for patients and staff alike.

A successful candidate would be self motivated, have experience in quality improvement work, be a strong and considerate team player.

Ideally you would be able to work 3 or more days but other agreements may be made for the right candidate. We can offer some flexibility in working hours to fit with a healthy work / life balance, but being employed by the health board can offer enhanced job security and other benefits such as indemnity cover, study leave and parental leave entitlement in accordance with Board policies.

Childcare voucher and cycle to work schemes are also supported. Forth Valley is a great place to live. Some of the best countryside in Scotland is on the doorstep, plus easy access by road and rail to all major population centres, including Edinburgh, Glasgow, Perth and Dundee.

A wide range of sporting and recreational facilities are also on offer. Excellent, communication, listening and organisational skills are essential.

Applicants will be registered with the General Medical Council and the General Practitioner Register by the time of appointment.

Applicants are required to have evidence of recent continued academic and professional development. It is a requirement that all applicants meet the criteria for joining the NHS Forth Valley Performers List by the time of appointment.
